Your Sign-On Bonus May Cost You $80,000. Your PSLF Will Not.

Two things sit in a new Pennsylvania physician's paperwork that look similar and are treated completely differently. Getting them the wrong way round is expensive.
What disqualifies you
RFA 67-206 lists factors that make an individual ineligible, and the first one is broad:
"Having any outstanding service obligation at the time of application or during any part of the service commitment period … for health professional or other service to the federal, state, or local government or other entity (for example, National Health Service Corps ('NHSC') Loan Repayment Program obligation, NHSC Scholarship Program obligation, or a NURSE Corps Loan Repayment Program obligation) or other entity (for example, a recruitment bonus that obligates you to remain employed at a certain site)."
That parenthetical is the one nobody expects. A sign-on bonus with a stay-or-repay clause is an obligation to remain employed at a site, and so it falls inside the disqualifier.
★★ And what does not disqualify you
The RFA then says something unusually helpful:
"Note: For the purposes of the LRP, the Public Service Loan Forgiveness program is NOT considered a service commitment because it does not obligate you to remain employed at a certain practice site."
PSLF requires qualifying employment, but it does not bind you to one named site, and the Department has written that distinction down. So a Pennsylvania physician can be working toward PSLF and still apply for up to $80,000 of state loan repayment.
That is a genuinely useful compatibility fact and it is almost never surfaced anywhere.

So what is the actual trade?
If your offer includes a bonus with a stay-or-repay clause, you are weighing that bonus against up to $80,000 of state loan repayment, less whatever the odds are of a competitive award in a cycle you can reach.
We are lenders, and we are not going to tell you how to negotiate an employment agreement. What we can tell you is that both numbers are real money that affects what you can carry on a mortgage, and that the bonus is usually the smaller of the two.
Worth checking: whether the bonus is structured as an obligation to remain at a site at all. Not every sign-on payment is. That is a question for whoever is advising you on the contract, and it is worth asking before you sign rather than after.
★ The clause that bites after you have won
One more line from the RFA, because it changes behaviour:
"Leaving the practice site(s) worked at during the time of application. Failure to remain employed at the practice site(s) worked at during the time of application will result in automatic disqualification of the application, even if an award has already been announced."
So the application is tied to the specific site you were at when you applied. Changing jobs between applying and starting the commitment ends it, award or no award.
For a physician who is also buying a house, that is worth knowing: the two-year commitment and the thirty-year mortgage are both bets on staying put, and they are tied to the same employer.
How does this interact with buying a house?
A physician loan can close up to 150 days before your start date on a signed employment contract, where agency underwriting is built around a shorter runway. So the mortgage does not force you to rush the employment decision.

Find out three things: whether your bonus obligates you to stay at a site, whether you carry any NHSC or NURSE Corps obligation, and whether you are pursuing PSLF. The first two can cost you the program. The third cannot.
